I recently had a problem with Vanguard failing to complete a gift of shares to a charity. Because the Vanguard IT system is so opaque it took them about two weeks to figure out why the order had not executed - fyi the purported explanation was unenlightening. One would hope that the SEC would look into the various glitches in Vanguard's IT system.
