What's the worst thing an agent did on your PC? How did you get it back?

I keep seeing posts about Claude Code or Cursor deleting a folder or committing an .env, and I can’t tell if that’s rare or if most people just fix it quietly and move on. So has it happened to you? What was it doing when it went wrong, and what did you actually get back? Git, Time Machine, nothing? I’m building a small Mac app around this (it records what an agent changes so you can roll a folder back), which is why I’m asking. Not linking it here. I mostly want to know if this is a real problem or just a good headline.
