Codex added a bunch of stuff I never asked for

A few weeks ago Codex added a bunch of stuff I never asked for. Everyone says "just commit before every AI change," and fair, that works if you remember. But what about the times it doesn't work: .the agent overwrote or deleted changes you hadn't commit .a checkpoint/rewind restored the wrong thing, or missed some changes .a restore wiped your own manual edits the agent deleted untracked files Has anyhing like this happened to you? Which tool, what went wrong, and how did you get out of it And honestly, if it's never happened in months of heavy use, I'd love to hear that too.
