Have any startups here actually lost money because of AI? - I will not promote

Curious to hear from founders who’ve had AI involved in their product or operations. I’m particularly interested in cases where an AI system: - made an incorrect decision or prediction - generated something that caused a financial loss - failed unexpectedly in production - caused a customer/business loss that the company had to absorb Not talking about the cost of using AI itself; I mean actual financial loss resulting from an AI-related failure or mistake. If this has happened to your startup, what happened, and roughly how significant was the impact? Trying to understand how common this problem actually is among startups before going deeper into something I’m working on.
