Post-Mortems for Agent Runs

The agent burned five hours on a refactor that should have taken one. The first hour was fine. The second, it was rewriting a module nobody had asked it to touch. The third through fifth went to rolling back, re-planning, and producing the smaller diff we should have ended up with at the start. The work landed late. The team was annoyed. The lesson was sitting there waiting to be picked up. We did not pick it up. The work shipped, the agent moved on, and the same failure mode showed up two weeks later in a
