refactor(tests): verify Doctor preview plugin warnings (#148137)

* refactor(doctor): remove unused warning wrapper

Remove the unused per-lookup warning wrapper and retain the cached policy-hook
owner, private configuration stripping, and live suppression fallback.

Port the privacy test through the existing factory and retain preview warning
coverage. Original and candidate each passed 145 cases; the normal 30-check
Blacksmith gate passed, followed by a scoped-clean precommit P2 review.

* chore(doctor): merge fixed main into warning cleanup

Merge b4c6e36eba8151fd42773567770af93e863c570a after the inherited
Docker fixture assertion failure. Main already contains the canonical
test repair in abbdd5287afc59899721285bca79fdfd8e6d289d.

The PR delta against that base remains the four-file warning-wrapper
cleanup. Preserve both existing histories without duplicating the repair.

* chore(doctor): merge canonical test routing repair

Merge 867e655d51d892c47ee68fc7f02c7e8ca0a5628d after the inherited
wizard fixture broker failure. The canonical upstream change restores
the wizard fixture's database-worker registration and keeps Radius
cases in their integration owner.

The PR delta against that base remains the four-file warning-wrapper
cleanup. Preserve both existing histories and upstream contributor credit
without duplicating the repair.

* chore(doctor): merge canonical QA fixture repairs

Merge 9d775cd38a7bc374bba5a84ede4ac8464a619b5b after the inherited
QA fixture failures in the prior composed CI checkout. The canonical
upstream fixes preserve process methods in simulated Bun fixtures and
use the existing test API for child fixtures with failure diagnostics.

The PR delta against that base remains the four-file warning-wrapper
cleanup. Preserve both histories and upstream contributor credit;
these already-landed repairs receive no additional cleanup credit.

* test(doctor): extract preview agent fixtures

Move the typed agent-roster and message-policy fixtures into a test-support
module without changing preview cases or warning and configuration-privacy
assertions.

* chore(doctor): merge canonical CI planner and fixture repairs

Merge 5764fa493344c0a041e0bc8319c07585d649b18e after the three
inherited failures in the previous composed CI checkout. Preserve the
canonical planner lint repair, transport fallback assertion, and labeler
fixture update with their existing upstream history and contributor credit.

Retain the five-path Doctor warning-wrapper cleanup and pure preview
fixture extraction. No additional cleanup credit is claimed for incoming
main, and local focused proof is not Linux transport proof.

* test(doctor): cover preview warning-hook forwarding

Assert that extra account warnings from both configured channels reach Doctor preview output. Move the existing pure agent fixtures into test support so the preview file shrinks while retaining all current cases.

Preserve the original PR ancestry and the runtime cleanup already present on main.

Integrate main 56c4d483375, including CI fixture repair 54c312971a4.
Preserve the reviewed two-file test contribution and all 16 proven source
inputs byte-for-byte; the current inventory remains 47 preview cases.
Reuse the completed regression control, 47-case run, changed checks, and
scoped P2 review. New exact-head CI follows publication.
