Claude sabotaging your project

I frequently see people claiming that Claude is sabotaging their work or project. Does this happen a lot? Can you give examples? I want to know what to be on the lookout for. Right now, I try to keep my directions very specific and tightly controlled. Does this happen when you give the system too much discretion?
