Most pet apps die because they ask you to type. Mine starts from one photo.

Not a vet. Just someone who got tired of answering "when was the last vaccine?" with "uh." What I now keep on my phone so a new clinic or a sitter is not guessing: - Chip number + last rabies date + valid-until - Current meds and the actual dose time (not "twice a day") - Weight from the last two months, not from memory - Open symptoms in one line ("drinking more since Tuesday, ate half a bowl yesterday") - One photo of the thing I'm worried about — stool / ear / eye — taken in daylight - For a sitter: the usual day, what a bad day looks like for this animal, and who to call. No full medical history. They don't need it and I don't want it floating around. The thing that actually made me keep doing this: if it takes more than a tap, I stop. Forms die on a Tuesday. I built an iOS app around that (TailStory — I'm the founder, so here's the disclosure). One pet is free. It is not a vet and it will not diagnose.
